Birth Certificates
Births occurring in Penetanguishene are no longer registered with the municipality.
You may apply for a new or replacement birth certificate or a certified copy of birth registration through Service Ontario. This service allows you to electronically submit the statement of live birth and submit applications for a birth certificate and social insurance number at the same time. Please note there will be a fee, payable by credit card only.
If you choose to mail your application or pay by cheque or money order, hard copy applications for birth certificates or newborn registration may be obtained within the Corporate Services department, located at Town Hall, or at a Service Ontario Centre.
If you need to travel with your newborn and have not yet received a birth certificate, please contact the Office of the Registrar General at 1-800-461-2156 or visit Service Ontario.
Death Registrations
For deaths that occur in Ontario, the funeral director or person responsible for completing the death information brings the documents to the Corporate Services department at town hall. The division registrar records the information, issues a burial permit and forwards the documents to the Office of the Registrar General.
When this has been completed, you may apply for a death certificate online through Service Ontario, payable by credit card only. If you choose to mail your application or pay by cheque or money order, hard copy death certificate applications may be obtained within the Corporate Services department, located at town hall, or at a Service Ontario Centre.
